*Must include at least one child younger than 19 and one adult guardian in same household.
Enrollment Fee
Every membership requires a $75 one-time enrollment fee for record review and to build a new patient chart.
If you leave the practice, then wish to return, a Re-enrollment fee of $200 is required.
Snowbirds can pause their membership over the winter if they plan on getting care elseware. There will be a $25 fee to review outside records on return.
Home Visit Fees
Home visits are availible within a 40 minute drive from Wilton. There will be a $25-$50 fee depending on distance.
